Retatrutide is an investigational triple agonist hitting the GIP, GLP-1, and glucagon receptors at once. The phase 2 obesity results reported some of the largest mean weight reductions seen for an incretin-based therapy, which is why it is getting so much attention ahead of phase 3.

Worth being precise: this is still investigational, not approved, and phase 2 results do not always hold up at scale. The glucagon-receptor component is the interesting differentiator versus the GLP-1 and GIP/GLP-1 drugs already on the market.

The glucagon-receptor component is the part I am watching. It is the clearest mechanistic difference from the GLP-1 and dual agonists already approved.

The phase 2 magnitude is striking, but I am holding judgment until phase 3 at scale. Big phase 2 effects do not always survive.
